These are the ten things an inspector or board is most likely to ask for. Requirements vary by state; this is a starting point, not legal advice.

  1. 1. Supervision agreement

    Is your medical director agreement signed and reviewed in the last 12 months?

    Boards ask for the written medical director agreement first.

  2. 2. Standing orders

    Do you have signed standing orders for every procedure and provider?

    Required in writing in Texas (22 TAC 169.27); best practice everywhere.

  3. 3. Chart review sign-off

    Has your medical director reviewed charts in the last 90 days, in writing?

    Quarterly review is required for Texas laser hair removal and expected by most boards.

  4. 4. Good-faith exams

    Is a good-faith exam documented before each new patient’s first treatment?

    Required before treatment in Texas and California.

  5. 5. Licences and BLS

    Are every staff licence and BLS card current, with copies on file?

    Expired or missing credentials were among the most common findings in a New York City review of med spas.

  6. 6. Fridge temperature log

    Do you log your neurotoxin fridge min/max temperature every open day?

    CDC recommends a reading at the start of each workday, kept for 3 years.

  7. 7. Emergency kit

    Was your emergency kit, including hyaluronidase, checked in the last 7 days?

    Your complication response depends on it being there and in date.

  8. 8. Sterilizer spore tests

    If you have an autoclave, is a weekly spore test logged?

    CDC guidance calls for biological indicator testing at least weekly.

  9. 9. OSHA bloodborne pathogens

    Are your OSHA bloodborne pathogens plan and yearly staff training on file?

    A federal requirement for employers whose staff handle sharps (29 CFR 1910.1030).

  10. 10. Inspection-ready binder

    Could you hand all of this to an inspector within one hour?

    Records that exist but can’t be found count as missing.
