Why keep competency checklists
A licence shows someone is allowed to do a procedure; a competency record shows your clinic checked they can do it your way, with your products and your protocols. Medical directors, inspectors and insurers ask for both, especially after a complication.
How to use this checklist
- Complete it when someone joins, before they treat patients on their own, and again every year.
- Record how each skill was checked. Watching someone treat a patient is stronger evidence than asking them to describe it.
- Mark anything not yet competent with a plan and a date to recheck, rather than leaving it blank.
- Add each device and treatment you offer, and cross out what doesn't apply.
Questions
Who should sign off competency?
Your medical director or a qualified clinician they delegate to. What your state allows each licence to do, and who may supervise them, varies, so check your state's rules.
How often should competency be reassessed?
Annually is common, and whenever someone starts a new treatment, you add a new device, or after an incident. Your medical director may set a different schedule.
