Why log complaints

A complaint log shows that complaints are heard, handled and closed. It also shows patterns: the same treatment, provider or step causing trouble again and again. And if a complaint turns into a board complaint or a claim, a dated record of how you responded matters.

How to use this log

  • Add every complaint, even ones resolved on the spot.
  • Mark clinical complaints, such as results, side effects or complications, and send them to your medical director.
  • Record what you did and when, and the date the complaint was closed.
  • Review the log monthly for patterns, and note any changes you make.

Questions

Should online reviews go in the complaint log?

Yes, when they describe a real problem. Log the review, what you did about it, and any private follow-up. Be careful not to confirm someone is a patient in a public reply.

Do HIPAA rules require a complaint process?

HIPAA-covered providers must have a process for privacy complaints and document them. This log works for those too, alongside service and clinical complaints.
